01428 651040 (See below for branch opening times)
If you have an emergency please ring the switchboard number 01483 783000 and the operator will connect you to the emergency line.
The phones are very busy for the first hour of the morning and again in the first hour of the afternoon so we do ask patients who are calling for results or non urgent enquiries to call outside these times.
At lunchtimes and when we are closed there is a message on the ansaphone to direct patients with urgent problems. This machine does not take messages.
We now have the facility for patients to book their appointments over the phone without needing to speak to a receptionist. This system can be used at any time of the day or night and particularly helps alleviate the morning rush for appointments. Haslemere patients should ring their reception number as usual. Fernhurst patients will need to ring one of the Haslemere direct dial reception numbers and then they can also access the system and make an appointment at Fernhurst.

APPOINTMENTS
Routine appointments with the doctors can be booked up to 8 weeks in advance. We also run a daily emergency clinic for very urgent problems.
Nurse and health care assistant appointments are usually pre booked and in order to give the appropriate amount of time in the appropriate clinic the patient will be asked for some indication as to the nature of the appointment
It is also possible to have a telephone appointment with the doctor where you will be given a time to speak to the doctor to discuss your problem.
HOME VISITS
If you are too ill to attend the surgery you may request a home visit which will normally take place after the morning surgeries are finished. It is important to ring before 11am otherwise the visit could be delayed. It is possible the doctor will phone you before he makes the visit.
OUT OF HOURS COVER
When the surgery is closed emergency care is available through Thamesdoc and your first approach must be by phone on 0300 130 1305. This number is also on our ansaphone during out of hours periods. Thamesdoc provide emergency surgeries at Haslemere Hospital, by appointment only, phone advice and home visits.
REPEAT PRESCIPTIONS
With your doctors agreement you may obtain repeat prescriptions with a repeat prescription card. When your medication is put on repeat a re order form will be issued to you with your prescriptions. When you require more medication you should tick the items you need on the form and bring or send it to us. Requests for medication not on your reorder slip must be made in writing for reasons of accuracy. We do not take requests by phone but faxed requests are accepted. There is also the facility on our website to order your prescriptions. Prescriptions are normally available 2 working days after they reach the surgery, whichever method you use to order them. However should you wish to use the collection service offered by the local chemists or send a stamped addressed envelope for your prescription then extra time will need to be allowed.
CONFIDENTIALITY
All patient data is treated with confidentiality by all members of the practice team. It may be disclosed to other health care professionals where this is deemed to be in the patients’ interest but this would not be done without the doctors’ permission. The practice is registered under the Data Protection Act.
COMPLAINTS
Although we endeavour to do our best for patients at all times there may be occasions when you feel you have cause for complaint. There is an in house complaints procedure in place and a leaflet describing this procedure may be obtained from reception or the waiting room. Any complaints may be discussed in the first instance with the Practice Manager, Stephanie Fisk. To make an appointment with her please phone on 01483 783040. Complaints will of course be dealt with in the strictest confidence. COMPLAINTS FORM
